Demographic developments, rise in multimorbidity and increase in chronic diseases pose considerable challenges on the Austrian healthcare system. This is where the interdisciplinary research project Linked Care - funded by the Austrian Research Promotion Agency - comes in. By digitally linking relevant information from care documentation, medical and pharmacy software, communication channels between the professional groups involved in mobile care, support and therapy are optimized. The incorporation of international standards guarantees seamless integration into existing healthcare data structures.
